Check List

• most recent mortgage statement or coupon
• 1 month pay stubs
• last 2 years worth of W2’s
• insurance declarations page (it has the premium listed) or quote if purchase
• 3 months most recent bank statements (all pages)
• most recent asset statements (all pages) – such as 401K, stocks, bonds, life insurance
• name, address, phone number of present/previous property mgmt co & 12 months cancelled checks if landlord
• Copy of recent appraisal (if 6 months old or newer)
• Copy of drivers license or green card (front & back) and social security card (due to the US Patriot Act)
• phone # to human resources department
• Tax info

If divorced:

• Copy of divorce decree

If self employed:

• Copy of business license (present and previous years)
• Business card
• Letter from CPA stating they have done your taxes as self employed for a minimum of  two years, or letter from you stating you do your own tax returns

If you own an investment property:

• copy of rental agreement
• two years tax returns

If you own a condo or are purchasing a condo:

• Name of COA, name of property management company, property manager’s name, address, and phone number

If you are purchasing property:

• Escrow company name and phone number
• Name and phone number of real estate agent
